Hospitals Are Being Forced to Build AI Governance Alone

AI is moving into healthcare faster than regulation is catching up. Hospitals are left in a difficult position, as there is no single framework that clearly defines how these tools should be used in practice, and the policies that do exist are scattered across different levels of government and different types of institutions.

At the same time, most of these policies are not actually telling hospitals what they can or cannot do with AI. Instead, they focus on things like disclosure, documentation, and internal oversight. Organizations are expected to explain how AI is used, track its performance, and put governance processes in place. What is missing are clear rules about when and how these tools should be used in clinical settings.

That creates a problem. If regulators are not defining use, hospitals have to.

In practice, this means that adopting AI is no longer just a technical or clinical decision. Health systems have to decide what use cases are acceptable, how models should be validated, who signs off on deployment, and how systems are monitored once they are in use. These decisions are becoming structured and visible, not informal or one-off.

This shift is not accidental. The current policy landscape is fragmented, and most of the instruments shaping AI in healthcare are advisory rather than binding. They set expectations without fully specifying implementation. At the same time, many of the concrete responsibilities in these policies fall on providers and health systems. The result is that hospitals are left to translate high-level guidance into something that actually works on the ground.

The operational impact is already visible. Hospitals deploying AI are starting to build governance infrastructure. That includes oversight committees, documentation standards, and review processes that did not exist a few years ago. Decisions that used to happen informally now need to be justified and, in some cases, audited.

This also changes how vendors are evaluated. Health systems are not just asking whether a model performs well. They are asking whether it fits into their governance structure. That raises the bar for documentation, explainability, and lifecycle management, even when regulation itself is still evolving.

The broader point is that AI governance in healthcare is not being defined solely by regulators. It is being built inside organizations. Hospitals are becoming the place where policy is interpreted, applied, and enforced.

That creates both risk and opportunity. Different systems may end up with very different standards, but it also allows governance to reflect real clinical and operational needs. For now, the key reality is simple. The success of AI in healthcare depends less on having clear external rules, and more on whether hospitals can build governance systems that actually work.
